PGSA can copy records from microfilms purchased from the Church of Jesus Christ of the Latter Day Saints for three large Chicago Polish churches:
Search the Marriage Index for Polish Parishes in Chicago through 1915, print the results, and highlight the name(s) of interest. If you are requesting birth records, search the St. John Cantius Church Births 1893 - 1915 Index or the members only Birth Index, print the results, and highlight the name(s) of interest. The public version of the latter database does NOT provide enough information to locate the record and requests made from it will be rejected. Include a donation check for $6.00 U.S. (members) and $10 U.S. for non-members per highlighted name. Donations are used for the costs of supplies, equipment, and materials.
